Job Description
The Care Clinic Dental Hygienist is responsible for delivering professional dental hygiene services to patients in the Clinic Dental Program, working under the administrative supervision of the Dental Hygiene Director and Chief Dental Officer while upholding the ASQ (Access, Stewardship, and Quality) pillars. The hygienist assumes responsibility for the hygiene program's goals, monitors progress, and implements changes to help meet them. Provides quality dental hygiene care to the patient population and makes appropriate referrals as needed. The Dental Hygienist performs related duties as assigned by the Dental Hygiene Director and the Chief Dental Officer. Performs dental measures designed to preserve teeth and prevent the spread of oral disease.
Essential Functions:
Provides dental hygiene services under the supervision of the Dental Hygiene Director and Chief Dental Officer, following established protocols, including, but not limited to, conducting daily dental hygiene clinic, establishing patient histories through interviews, and providing oral hygiene education.
Assists all Integrated Care Clinics by performing triage, screening dental radiographs, and preparing the patient for the dentist.
Ensures appropriate documentation and maintenance of dental records.
Performs Case Management: thorough and effective explanation of treatment plans, including scheduling; referral to Alliance Dental Practices; follow-up on treatment plans
Maintains compliance with Occupational Safety and Health Administration regulations and assists in accordance with those guidelines.
Responsible for maintaining knowledge of such regulations and for the application of such guidelines in the workplace.
Acts as a representative of Summit Community Care Clinic with local dentists and their staff, dental schools, dental laboratories, service clubs and organizations, patients, and the general public, acting in a professional and responsible manner.
Understand and perform Case Management to ensure the completion of treatment planning.
Maintains emergency kit and performs other related job duties as determined by the Lead Dental Hygienist and Chief Dental Officer, Clinic OSHA Leader, and the Cultural Competency Team.

Education/Experience:
Must-Haves to be successful:
Licensed in Colorado.
Current CPR certification.
Current Anesthesia certification by the State of Colorado or within one year of employment.
Knowledge of Healthcare settings, preferably in a non-profit.
Knowledge of Federal, State, and institutional regulations and guidelines for the provision of dental outpatient services.
Ability to independently assess the nature and extent of dental, periodontic, and associated conditions and pathology in dental outpatients.
Knowledge of patient screening, charting, medical history procedures, and documentation.
Ability to communicate technical information to non-technical personnel.
Ability to perform a full range of established dental hygienist protocols and procedures.
Ability to analyze medical records and prescribed protocols to formulate effective therapeutic treatment plans.
Knowledge and understanding of dental surgery and associated procedures, terminology, and techniques.
Ability to set up and prepare dental equipment and instrumentation in accordance with established regulations and guidelines.
Solid competency in digital systems (including Microsoft Office Suite) required
Critical level of attention to detail, organizational skills, and time management
Ability to define problems, establish facts, and draw conclusions (evidence-based decision-making skills)
Nice to Have:
At least one (1) year of experience as a Dental Hygienist in a dental office setting is preferred.
Ability to read, write, and clearly speak English and Spanish is highly preferred
Work Environment:
The work environment is in a healthcare setting and can be fast-paced, involving direct work with patients and healthcare professionals. The individual may also work in a clerical or school setting. About Elevated Community Health
The mission of Elevated Community Health (ECH) is to provide exceptional, integrated, patient-centered health services designed to meet the needs of all patients, particularly those who experience barriers to accessing care, regardless of their ability to pay. Our goal is to improve the health and well-being of our patients by preventing illness and treating disease in the most comprehensive and compassionate way possible. Elevated Community Health was established in 1993 as a one-night-a-week walk-in clinic staffed entirely by volunteers and managed by Summit County Public Health. Over time, we added extra evening hours and eventually daytime hours. In 2006, we became an independent nonprofit and relocated from Summit County Public Health to our current space. In 2013, ECH received its Federally Qualified Health Center Look-Alike status and became a full Federally Qualified Health Center in 2015. Over time, our services have grown to include medical, reproductive health, oral health, counseling, physical therapy, and group preventative health services.
