Today a seven-day-a-week, multi-site community health center, Zufall provides comprehensive services include adult and pediatric medicine, dental care, women’s health, cancer screening, podiatry, HIV care, neurology, optometry, behavioral health, nutrition, and clinical pharmacy counseling. They are delivered by a caring, bilingual staff of board-certified physicians and dentists (paid and volunteer), social workers, pharmacists, and support professionals. Zufall also offers enabling services including eligibility assistance, case management, transportation, health literacy training, outreach, and translation. As one of the first medical practices in New Jersey to earn designation as a Patient-Centered Medical Home, Zufall was also a leader in converting to electronic health records. Moreover, Zufall was one of the first in the state to focus on improving population health and have had great success supporting wellness through programs that advance good nutrition, health literacy, education, exercise, and health insurance enrollment for our patients and the community-at-large. Zufall has gained national and regional attention, receiving numerous awards for quality, performance, and innovation.
