Job Description
Always Home care is looking for a team player who’s bright and able to multi-task to join our growing agency as Spanish Speaking Intake Coordinator. The office location for this position is located in Brooklyn, NY 11235.
JOB RESPONSIBILITIES
Receive referrals from referral sources and input information into Referral Management
Verify patient’s insurance to determine eligibility
Educate patients on services we provide as well managing the process of converting a referral into an active home care case.
Ensuring referral forms are completed correctly, and assisting with securing any supporting documentation needed.
Send referral to MLTC for potential enrollment or Medicaid assistance
Follow up with patient's request for increase of hours
Acting as a point of contact with contracted vendors to build and flourish with these contracts to increase agency census
Accepting incoming referrals and entering them into tracking software.(HHA Exchange and Bolt)
Send referral to appropriate CHHA for skilled services as well as follow up with hospitalized patients to see if these members may need CHHA services
Promptly advising appropriate staff of new admissions.
QUALIFICATIONS:
1 year of experience in home health care intake a must
Understanding the requirements for CHHA’s and MLTC’s
Understanding of communicating effectively with employee, patients and their families, medical and community affiliates in order to develop positive relationships.
Organized and punctual
Proficient in Microsoft Office Suite, HHA Exchange
Investigative ability, highly organized, self-motivated, takes initiative, excellent written and verbal communication and analytical critical thinking skills; able to perform without much supervision.
Energetic, positive, and self-motivated.
Must be bilingual (Only apply if you can speak both Spanish and English)
WORK SCHEDULE:
Full Time Office hours
Monday – Friday 9AM to 5PM
