Our Program of All-Inclusive Care for the Elderly (PACE) brings together a passionate interdisciplinary team to provide personalized, wraparound care that honors each participant's dignity, preferences, and goals.

We're looking for a Home Care Coordinator (HCC) who thrives on building relationships, solving problems, and making sure our participants get the right care in the right place — most often, their own home.

What You'll Do
As an HCC, you'll be a core member of our Interdisciplinary Team (IDT), bridging the gap between our PACE Center and our participants' homes. Day to day, you'll:
Conduct initial, annual, and as-needed nursing assessments to inform participants' plans of care
Visit participants in their homes to evaluate physical and social needs and coordinate appropriate supports
Develop, update, and close care plans alongside the IDT
Coordinate personal care services and ensure full coverage across all cases
Serve as the primary liaison with our contracted Home Care Agency
Monitor and document homecare services accurately and in a timely way
Assist with investigations and resolution of homecare complaints
Provide assessments and care at the PACE Center, in participants' homes, and at Assisted Living Facilities
Participate in IDT meetings, staff meetings, and in-services
What You'll Bring
High School Diploma or GED (required)
At least one year of experience working with a frail or elderly population
Strong written and verbal communication skills
Ability to work both independently and as part of a collaborative team
Solid computer skills (documentation, scheduling, EHR systems)
A reliable personal vehicle (may be required for home visits)
Commitment to HIPAA, confidentiality, and professional standards

Physical Requirements
This role requires moving throughout the facility and participants' homes throughout the workday. You should be able to lift up to 35 lbs occasionally, 15 lbs frequently, and 7 lbs constantly (with assistance required for anything over 25 lbs). The role involves frequent sitting, standing, walking, and repetitive movements, with occasional bending, reaching, squatting, and kneeling.

Important Note on Health Clearance
Per CDC Category I exposure guidelines, all HCCs must be cleared for communicable disease and provide proof of up-to-date immunizations prior to providing direct participant care.
