Job Description

Job Summary
The Patient Care Coordinator is a key member of our clinical support team, responsible for ensuring a smooth and positive experience for patients throughout their physical therapy journey. This role involves managing patient schedules, communicating with healthcare providers, handling administrative tasks, and supporting clinical staff to deliver exceptional patient care.

Duties
Patient Communication: Serve as the primary point of contact for patients, answering inquiries, scheduling appointments, and providing clear information about services and insurance coverage.

Scheduling Management: Efficiently schedule and coordinate patient appointments, follow-ups, and cancellations using the practice’s electronic scheduling system.

Referral Coordination: Collect and process patient referrals, communicate with referring physicians, and ensure all necessary documentation is received and entered into patient records.

Administrative Support: Maintain accurate patient records, manage documentation flow, and assist with data entry and filing.

Customer Service: Provide a welcoming environment for patients and visitors, addressing concerns and ensuring a positive experience at every interaction.

Team Collaboration: Work closely with physical therapists, assistants, and other staff to coordinate care and support the overall operations of the clinic.

Maintain confidentiality of sensitive patient information at all times.

Requirements
Education: High school diploma or equivalent required; associate’s or bachelor’s degree in healthcare administration or related field preferred.

Experience: Previous experience in a medical office or customer service role preferred; experience in physical therapy or rehabilitation setting a plus.

Skills: Strong communication, organizational, and interpersonal skills; proficiency with electronic health records and scheduling systems; ability to multitask and work in a fast-paced environment.

Certifications: None required, but medical office or billing certification is a plus.

Ability to work collaboratively in a team-oriented environment while managing multiple tasks efficiently. This role is essential for delivering high-quality care to our patients while ensuring operational excellence within the healthcare setting.

Physical Requirements
Ability to sit for extended periods.

Occasional standing, walking, and lifting up to 40-50 pounds.
