ABOUT THE ROLE
We are hiring a Staffing Coordinator to run our daily field scheduling and coordinate our contracted therapy providers and their documentation. This is a hands-on, high-volume, phone-heavy role at the center of daily operations. You will work closely with our Administrator, Director of Nursing, and care coordination team.
WHAT YOU'LL DO
Staffing & Scheduling:
Coordinate and schedule visits for RNs, LVNs, and Home Health Aides (CHHAs) based on physician-ordered frequencies
Match clinician availability, skills, language, and geography to patient needs
Manage schedule changes, call-outs, and urgent coverage needs; communicate changes to field staff and patients promptly
Enter and maintain schedules in our EMR (WellSky/Kinnser); monitor missed visits and follow up same-day
Track visit utilization against authorizations for managed care patients
Contract Therapy Coordination & Documentation:
Assign PT, OT, and ST referrals to our contracted therapy providers and confirm timely starts of care
Track therapy visit frequencies against the plan of care; follow up on missed, late, or unconfirmed visits
Collect therapy visit notes, evaluations, and re-evaluations from contract therapists; follow up persistently on outstanding documentation
Upload therapy documentation into WellSky/Kinnser accurately and on time
Reconcile therapy visits performed against contractor invoices and flag discrepancies
WHAT WE'RE LOOKING FOR
1+ year of staffing, scheduling, or coordination experience in home health, home care, or hospice (home health strongly preferred)
Experience working with contracted therapy providers and chasing clinical documentation is a major plus — you know the grind and you're good at it
Familiarity with home health disciplines and visit types; WellSky/Kinnser experience a plus
Excellent phone presence and relentless follow-through — you will be on the phone with clinicians, contractors, and patients all day
Strong organizational skills and attention to detail; comfortable tracking many moving pieces at once without letting anything fall through
Bilingual English/Spanish or English/Armenian a plus
High school diploma or equivalent required; healthcare administration coursework a plus
